Violations included silverware stored improperly; open beverage containers in food-prep area; chemicals stored with food; hand sink blocked by carbon-dioxide tanks; food debris on mixer; dirty soda nozzles and reach-in cooler; unlabeled food containers in cooler; no thermometers in coolers; and uncovered trash can in restroom. Auntee M's Market, 4275 S. Arville St., received 14 demerits on April 10. A B grade was posted. Violations included raw eggs stored above milk in cooler; dirty soda nozzles; food products stored with chemicals under cash register; ice machine in disrepair; and dirty fan covers. BJ's Pizza, 6847 W. Flamingo Road, received 19 demerits on April 9. A B grade was posted. Violations included food stored in nonworking cooler; raw beef and eggs stored above cheese in cooler; microwave and soda nozzles dirty; covered trash can needed in restroom; and only one out of three coolers working. Carl's Jr. Restaurant, 4815 Spring Mountain Road, received 16 demerits on April 10. A B grade was posted. Violations included a box of hamburger patties placed on top of trash can while patties were being placed in broiler; onions and boxes of food stored on floor of walk-in cooler; open bag of salt in dry-storage area; no paper towels in employee's restroom; no trash can in prep area; wipe cloths not stored in sanitizer; inadequate lighting in walk-in cooler; unsecured carbon-dioxide tanks; and employees not washing their hands. El Pollo Loco, 4015 S. Decatur Blvd., received 19 demerits on April 11. A B grade was posted. Violations included unsecured carbon-dioxide tanks; missing ceiling panels; burned-out lights in hood; cutting board not smooth and cleanable; uncovered foods in walk-in cooler and dry-storage area; wipe cloths not stored in sanitizer; employees not washing hands; employees eating food on cook's line; no hand soap or paper towels at hand sink; three-compartment sink dirty; and a broken faucet on kitchen hand sink. Foothills Ranch restaurant, 3377 N. Rancho Drive, received 20 demerits on April 8. A B grade was posted. Violations included deli table not maintaining foods at proper temperatures; unlabeled foods in walk-in cooler; area under steam table and broiler dirty; raw eggs stored over ready-to-eat foods; raw steaks stored over onions and peppers; dried food debris on meat slicer; can opener, coffee maker and soda nozzles dirty; and uncovered foods in walk-in cooler. Granpa Flores, 3149 N. Rancho Drive, received 58 demerits on April 9. A C grade was posted and the establishment closed. Violations included deli cooler and reach-in cooler not maintaining proper temperatures; cook placing items in three-compartment sink to cool; no thermometers in any of the reach-in coolers; cook did not have a stem thermometer; raw meat and marinating meats stored above ready-to-eat food; ceramic bowls used as scoops; milk and half-and-half stored in ice machine; uncovered foods in reach-in cooler; wipe cloth used to cover cooked peppers; case of lettuce stored at room temperature on the floor; waitress and cook handling salads and raw shrimp, and preparing plates without washing hands between activities; no hand sink in kitchen; cutting boards worn and not cleanable; unlabeled foods in reach-in cooler; two employees did not have health cards; unclean surfaces throughout establishment; cockroaches observed at waitress station, by hand sink and in restroom; dirty walls and floors; and hand sink not plumbed properly. The establishment was reinspected on April 17 and received seven demerits. An A grade was posted and the establishment reopened. McDonald's, 3175 N. Rancho Drive, received 15 demerits on April 8. A B grade was posted. Violations included no soap or paper towels in men's restroom; two employees did not have health cards; eggs stored above ready-to-eat food; thermometer not working in burger freezer; gaskets in disrepair or missing on burger freezer; and dirty floors under some equipment.
Monorail Sushi, Bally's, 3645 Las Vegas Blvd. South, received 15 demerits on April 7. A B grade was posted. Violations included cooked chicken breasts stored at room temperature; chicken thawed improperly at room temperature; garbage can used to store food; no paper towels or soap at hand sink; and access to hand sink blocked by garbage can. Pin Kaow Restaurant, 1974 N. Rainbow Blvd., received 17 demerits on April 10. A B grade was posted. Violations included cooler not cooling to proper temperature; raw beef stored over ready-to-eat food; some employees did not have health cards; employees not wearing hair restraints; dishes not sanitized; wipe cloths not stored in sanitizer; and dirty equipment surfaces, shelves and racks. Sonic Drive-In, 300 S. Boulder Highway, Henderson, received 28 demerits on April 11. A C grade was posted. Violations included cups used to scoop ice; chemicals stored with food products; broken hand sink in food-prep area; crockpot used to heat gravy; unlabeled food containers; employees not wearing hair restraints; public walking through food-prep area to reach bathrooms; utensils stored improperly; test kit needed for sanitizer; wipe cloths not stored in sanitizer; inside of ice machine dirty; dirty vent fans in reach-in cooler; back door in disrepair; and some employees did not have health cards. Thai Garden Restaurant, 5600 Spring Mountain Road, received 19 demerits on April 11. A B grade was posted. Violations included chicken not heated or cooled to proper temperatures; sanitizer not used when washing dishes; dirty landing on sink; hoods, stoves, fryers, trays, interior of freezer, curtains and front curtain dirty; shrimp stored on dirty landing of sink next to dirty dishes; beer stored in men's restroom; ice scoop stored on top of dirty counter; wipe cloths not stored in sanitizer; and unapproved food- and nonfood-contact surfaces. Yan Chinese Express, 1756 E. Charleston Blvd., received 18 demerits on April 7. A B grade was posted. Violations included spoiled meat in walk-in cooler; foul odor in walk-in cooler; dirty food containers; cook sitting on table; no stoppers for three-compartment sink; cutting boards stored on floor; cockroaches observed in the establishment; and a plugged drain in the three-compartment sink. The following establishments have been reinspected: Capri Pizza Restaurant, 4080 S. Paradise Road, received five demerits on April 3 and was upgraded to an A. Horseshoe Club main kitchen, Binion's Horseshoe, 128 Fremont St., received one demerit on April 15 and was upgraded to an A. Napoli Pizzeria, 1650 W. Warm Springs Road, Henderson, received 11 demerits on April 4 and retained the B grade. On April 8, the establishment received 12 demerits and still retained the B grade. Nora's Cuisine, 6020 W. Flamingo Road, received one demerit on April 15 and was upgraded to an A. Ocha Restaurant, 1201 Las Vegas Blvd. South, received 33 demerits on April 7 and was downgraded to a C. Pablo's Cafe, Santa Fe, 4949 N. Rancho Drive, received nine demerits on April 4 and was upgraded to an A. Santora's Pizza & Subs, 1985 N. Nellis Blvd., received 10 demerits on March 27 and was upgraded to an A. Subway, 3180 E. Desert Inn Road, received eight demerits on April 3 and was upgraded to an A. Tanya Restaurant, 210 W. Sahara Ave., received four demerits on April 7 and was upgraded to an A. Time In Lounge kitchen, 6120 W. Tropicana Ave., received five demerits on April 7 and was upgraded to an A.
