Boston Pizza, 1507 Las Vegas Blvd. South, received 20 demerits July 26. Violations included diced and sliced ham, cheese, tuna and mayonnaise maintained at improper temperature in deli reach-in cooler. GRADE: B

Burger King, 1050 Nevada Highway, Boulder City, received 18 demerits July 25. Violations included no hot water at three-compartment sink and no warm water at either hand sink. GRADE: Closed because of substantial health hazard.

Cafe Modena La Pizzeria, 1725 E. Warm Springs Road, received 33 demerits July 25. Violations included an accumulation of black mold and slime on all soda nozzles. GRADE: C

Cafe Verdi West, 3111 S. Valley View Blvd., received 40 demerits July 27. Violations included a can of pesticide stored next to kitchenware on shelf. GRADE: C

China A Go Go VI, 10140 W. Tropicana Ave., received 20 demerits July 26. Violations included food handler sneezed into hand and continued working without washing hands. GRADE: B

Cozymel’s Coastal Mexican Grill prep kitchen, 355 Hughes Center Drive, received 38 demerits July 24. Violations included frozen calamari being thawed at room temperature. GRADE: C

Dairy Queen, 2607 Windmill Parkway, Henderson, received 28 demerits July 27. Violations included kitchenware being rinsed in floor sink and clean kitchenware stored in dirty milk crate. GRADE: C

Del Taco, 898 S. Boulder Highway, Henderson, received 36 demerits July 23. Violations included an accumulation of moldlike substance on nozzles of both soda machines and interior of ice machine. GRADE: C

Einstein Bros. Bagels, 7541 W. Lake Mead Blvd., received 15 demerits July 23. Violations included no sanitizer in three-compartment sink or on wiping cloths. GRADE: B
