Dr. Sean Mohtashami wants to leave a legacy and plenty of smiles along the way to change people’s lives.
The founder of the 4M Institute in Long Beach that trains dentists in the latest techniques of dental implants, Mohtashami is the CEO of 4M Dental Implant Centers that includes an office in Las Vegas and eight others in Southern California in Long Beach, San Diego, Murrieta, Riverside, Newport Beach, Anaheim Hills, Valencia and Palm Desert. The offices also do other dental procedures.
Mohtashami, who started 27 years ago as a general dentist in Las Vegas, has invested hundreds of thousands of dollars on his own education, traveling across the country and around the world to learn and eventually led to him launching high-end implant centers. Mohtashami has been doing implant surgery for the past 20 years and has spent a lot of time perfecting the full-arch procedure he refers to as the 4M procedure.
“What I discovered through my journey was a massive, unmet demand in the market,” Mohtashami said. “Millions of patients need full arch solutions, but there simply aren’t enough clinicians equipped to handle these cases predictably and profitably. Through my own practice, I’ve been able to perfect a model that not only delivers exceptional patient care but also creates sustainable practice growth.
“This wasn’t theoretical learning – it was real-world experience gained through treating thousands of cases, managing complications, and continuously refining our protocols,” Mohtashami said. “Every challenge we faced became an opportunity to improve our methodology. We’ve taken what works, systematized it, and now we’re democratizing this knowledge for the entire industry.”
Full-arch restoration isn’t just a procedure but an art form that requires precision, expertise and comprehensive understanding, Mohtashami said. When a patient comes to a center for the full arch treatment, they’re not just asking for new teeth, “they’re entrusting us with their health, their confidence and their future quality of life,” he added.
“The complexity of these cases demands excellence at every level,” Mohtashami said. “From the initial consultation to the final restoration, there are countless critical decisions and techniques that can make the difference between success and failure. And in full-arch cases, we simply cannot afford to have redos – the stakes are too high, both for our patients and our practices.”
Mohtashami said he had so much success that five years ago built his institute in Long Beach to teach the procedure to other dentists.
“I wanted to build a profound legacy that has an impact on more than just what I do personally,” Mohtashami said. “I wanted to have a bigger impact on other dentists and doctors, and the only way I could do that was by teaching it. We’ve more than 100 graduates from the 4M Institute who learned the procedure and a good number go back and use this, and also change the lives of patients through this transformation. It makes me happy to know that I’ve been able to pass on my skills and knowledge to other dentists.”
The centers serve patients with no teeth or teeth that they’re told can’t be saved and must be removed, Mohtashami said. In one setting at his center, he said they will place a minimum of four implants in their jawbone and build a permanent set of teeth on top of those four implants.
“They walk into our practice with no teeth or terminating teeth, and that same day they go home with a new set of permanent teeth,” Mohtashami said.
There are 60 million Americans who wear dentures and others who will get them, and Mohtashami said he saw that demand that prompted him to expand his practices from Las Vegas to Southern California.
“Las Vegas is still my baby and my original practice that I started in 1998,” Mohtashami said, adding he continues to handle the surgical impacts in the valley.
Mohtashami said his centers are able to treat patients that most other doctors will turn away such as losing a lot of bones over the years. They are candidates because there are other procedures designed for patients without much upper jawbone left, he said. They’re called zygomatic and pterygoid procedures.
“They can have a permanent set of teeth with implants, but it just requires a different level of expertise,” Mohtashami said.
Mohtashami said his centers are different by focusing on exceptional care and innovation, and their doctors are part of the teaching community. He has five surgeons among the nine centers in addition to dentists who do day-to-day procedures.
“We’re all the leaders in this field because we constantly teach other doctors on what to do and how to do it,” Mohtashami said.
Mohtashami said those considering the procedure should ask themselves such questions: Are they missing teeth, wearing dentures, have gum disease, have loose teeth or face consequences of problems with their teeth, such as having difficulty getting a promotion and having a social life.
“If you fall into any of these categories, then this procedure can completely change your life,” Mohtashami said. “We see it every day. Patients that are prohibited from so many daily activities. Grandparents are embarrassed to take pictures with their grandkids. There are middle-aged people who can’t get a job and younger people that can’t go out on a date.
