Chicago Joe's restaurant, 820 S. Fourth St., received 12 demerits Aug. 22. Violations included nonapproved reach-in cooler and chest freezer in use. GRADE B

Chinese Village Lounge, 608 N. Rainbow Blvd., received 19 demerits Aug. 23. Violations included employees changing tasks without following proper hand-washing techniques. GRADE B

Chinese Village Restaurant, 608 N. Rainbow Blvd., received 20 demerits Aug. 23. Violations included unlabeled chemicals stored above and next to food and single-service items. GRADE B

Cold Stone Creamery, 1311 W. Sunset Road, Henderson, received 30 demerits Aug. 24. Violations included no sanitizer in three-compartment sink or sanitizer bucket. GRADE C

Delish Cafe, 2550 Anthem Village Drive, Henderson, received 18 demerits Aug. 23. Violations included deli and make-table reach-in coolers not maintaining proper temperatures. GRADE B

Playas Del Oro, 3210 E. Lake Mead Blvd., North Las Vegas, received 20 demerits Aug. 23. Violations included hand sinks filled with utensils and empty tomato juice cans. GRADE B
